Aftercare Programming: Assertive Continuing Care (ACC)
Relapse rates of 60% to 75% are not uncommon in the first 30 days following discharge from residential treatment. Relapse rates as high as 30% have been observed in the first 72 hours following successful completion of residential treatment and this can be associated with the lack of support which is common when waiting for the first scheduled appointment after residential care.
In order to reduce the risks that lead to relapse, the Dave Smith Youth Treatment Centre has assembled a special outreach team of Assertive Continuing Care (ACC) counsellors who engage youth and families shortly after residential discharge (often within 24-48 hours). The ACC program provides intensive case management services to connect the adolescent and family to appropriate community-based treatment and related services – based on specific client needs.
By assigning an ACC counsellor to our clients at admission we provide a critical link during those first few days following residential care. . This timely connection allows the ACC counsellor to provide immediate support to a client, even in the event of an unplanned discharge.
The ACC aftercare program extends for up to 12-weeks post-residential discharge.
